Host connectivity on a VMFS volume is permanently lost.
This alert can occur after a lengthy loss of connectivity from the storage, possibly caused if the heartbeat slot used by the host for the volume is repeatedly cleared and claimed by another host.
All queued I/O to the volume is discarded and any virtual machines should be shut down.
Reboot the affected host, after first vMotioning running VMs to an alternative host.
Check for storage connectivity issues. For more information, see VMware article Troubleshooting ESX and ESXi connectivity to fibre channel arrays (1003682).
